Personal Data Request Form

Under Article 11 of the Turkish Personal Data Protection Law No. 6698 (the "KVK Law"), personal data subjects (the "Applicant") are granted the right to make requests regarding the processing of their personal data. Pursuant to Article 13/1 of the Law, applications to our Company as data controller must be submitted in person, in Turkish and in writing, using the methods below as determined by the Personal Data Protection Board.
Application Methods
- Written application in person — to Akdeniz Üniversitesi Akdeniz Tekno Kenti K:2 No:3 Konyaaltı / Antalya, Türkiye; write "KVKK Kapsamında Bilgi Talebi" (Information Request Under the KVKK) on the envelope.
- Application via notary — to the same address; write "KVKK Kapsamında Bilgi Talebi" on the envelope.
- Via registered electronic mail (KEP) — to resatmendiotelcilik@hs01.kep.tr; write "KVKK Bilgi Talebi" in the e-mail subject line.
- Via an e-mail address registered with our Company — to kvkk@rmosyazilim.com; write "KVKK Bilgi Talebi" in the e-mail subject line.
Information Required in the Application
Pursuant to the Communiqué on Application Procedures and Principles (Official Gazette No. 30356, 10.03.2018), applications must include:
- Name, surname and — if the application is written — signature,
- Turkish ID number for citizens of the Republic of Türkiye; nationality, passport number or ID number (if any) for foreign nationals,
- Residential or workplace address for notification,
- E-mail address, telephone and fax number for notification, if available,
- The subject of the request.
Information and documents related to the subject must be attached to the application. Our Company reserves the right to request additional documents and information (copy of ID card or driving licence, etc.) for identity and authority verification. If the information provided is inaccurate or outdated, or an unauthorised application is made, our Company accepts no liability arising from such applications.
Response Time
Pursuant to Article 13/2 of the KVK Law, your application will be answered free of charge in writing within thirty days at the latest, depending on its nature. No fee is charged for replies of up to ten pages; for each page above ten, the transaction fee determined by the Board (1 TL) applies.
